If the changes have been made and the VTF is in the Addons folder, there shouldn't be anything wrong... Maybe if you extract the files from the VTF and copy them into the games main directory (left4dead\materials\vgui)?

Search the L4D2 section for 'Last Names', it'll bring up several mods that change the survivors names. There are none for L4D1, but all you need to do is find the file 'closecaption_english'in the first games folders and copy the names into there.
