This mutation is what I believe realism and classic zombie style should be.

realism
no crosshair
less ammo
infinite/faster incaps
no rescue
no glows
CI only
no random mobs
pistols are primary
2x infected damaged
start with nothing

hardcore
no ammo stash
no defib/med kits
revive less hp

To start a campaign with a mutation type in console 'map mapname mutation'
